Insurance Information & Billing

Appointment Details

Each appointment includes consultation, evaluation & assessment, treatment, home care instructions, billing, and scheduling. A minimum of 24 hours' notice is required for cancellations. If this notice is not provided, the full cost of the appointment will be the patient’s responsibility. Missed appointment fees cannot be billed to third-party insurers (e.g. MSP, ICBC, or extended healthcare providers).

Rates & Payment

The rates for our services typically range anywhere between $73-140. For a comprehensive list of our services and their rates, please click here!

We accept cash, cheque, debit card, and credit card. We are happy to discuss payment plan options as well. It is our policy that payment is made at the time of service.

Extended Health Insurance Plans

At Blue Oak Physiotherapy, we accept plans from all major insurance providers, as listed below.

We know insurance can be confusing. As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.

Typically, patients must pay upfront and submit their receipts for reimbursement. However, we do offer direct billing to all major insurance providers. To access direct billing, patients must complete and sign a consent form before their appointment. For all other insurance providers, patients will need to submit a receipt/invoice (PDF or hard copy) to their insurer. A fully detailed receipt will be provided at the time of your visit.

Is your insurance provider not listed? Please call the office at (604) 825-3622 to verify if we accept yours.

Private Pay Plan

Our Abbotsford clinic also offers a private pay plan for those patients who do not have health insurance or those who have maxed out their benefits. Blue Oak Physiotherapy offers a $85.00 per visit, self-pay fee.

ICBC Coverage

If you have an active ICBC claim number, you are entitled to 12 treatments within 12 weeks of your accident. For treatment beyond 12 weeks, authorization from your ICBC adjuster is required before coverage can be approved. If your accident occurred more than 12 weeks prior to your first appointment, you must obtain ICBC authorization before we can bill ICBC directly.
